Insurance/Community Health Choices (CHC) Enrollment
The Pennsylvania Medicaid Waiver Program provides Medicaid recipients with long-term care services outside of institutional settings. The program provides home and community-based services to eligible individuals who are elderly, disabled or have chronic medical conditions. The waiver program is a way to get the care you need while remaining in your home or community. The program offers different waiver programs to meet the needs of different populations. The Pennsylvania Medicaid Waiver Program provides a valuable service to individuals who require long-term care services but prefer to remain in their homes or communities.
Each of these waivers has specific eligibility criteria and offers different types of services.
The services provided through the various waiver programs include:
- personal care
- respite care
- home health care
- transportation
- assistive technology
The combination of services offered will depend on which waiver program covers you. These services allow individuals to remain in their homes and communities while receiving the care they need.
To be eligible for the Medicaid Waiver Program in Pennsylvania, individuals must:
- meet certain income and asset requirements,
- be a resident of Pennsylvania
- have a medical need for long-term care service
To enroll in CHC, follow the following steps:
Step 1
Check if you qualify for Medical Assistance. Use the COMPASS website to check your eligibility.
Step 2
If you are eligible, register for the state of Pennsylvania’s Health and Human Services programs on the COMPASS website.
Step 3
Within 60 days, a case worker from a local County Assistance Office (CAO) will be assigned to review your information.
Step 4
Once you are approved, you will be responsible for selecting one of the three managed care plans: Amerihealth Caritas, UPMC, and PA Health and Wellness.
Step 5
If you qualify for Pennsylvania’s Medicaid waiver program, you will be notified which services you qualify for upon your CHC program approval.
Step 6
Choose Renaissance Home Care as your appointed home care agency and begin receiving care at home.
